In order to attend Little Flower Catholic School we require shot records to be updated per the state of Maryland Regulations.  Click here to see the current state regulations.

We also require the state health inventory to be completed by your family physician.  To download this document click here.  In addition people who live in certain zip codes are required to be tested for lead levels, for that form (and list of zip codes) click here.
